Buurtzorg revolutionizes mobile care provision through a client-centered, resource-based and self-organized approach.
Solution
Buurtzorg revolutionizes the way in which mobile care is provided. Buurtzorg developed a client-centred model that tries to improve the self-reliance of clients by building on their personal resources and social networks. The care provision model is based on self-organized nurse teams. Nurse teams consist of 10-12 members and manage all key aspects of mobile care themselves: case assessment, care planning, care documentation, client acquisition, hiring of new team members. Teams are supported by a small group of coaches and administrative staff.
Impact
The Buurtzorg model drastically improved client and employee satisfaction as well as quality of care. Currently 9000+ nurses are organized in Buurtzorg teams throughout the Netherlands. Each team serves 5000-10000 clients in their communities. The model already has been transferred to Germany, Japan and other countries.
